The SHAILI Mobile Application is a public Health Initiative of the Government of Kerala’s, specifically engineered to combat the rising incidence of lifestyle-related diseases. Developed by the State Digital Health Mission - eHealth Kerala under the aegis of the Nava Kerala Karma Plan 2.0, this initiative digitises the annual population-based health screening programme. It empowers ASHAs with a robust digital tool to execute household-level interventions for citizens aged 30 and above, ensuring comprehensive health coverage and data accuracy across the state.
Functioning as a critical frontline diagnostic tool, SHAILI facilitates the early identification of Non-Communicable Diseases, including Hypertension, Diabetes, and common cancers like Cervical, Breast and Oral Cancer. Through the digital application of the Community-Based Assessment Checklist (CBAC), the system generates immediate health risk scores that drive intelligent population stratification. This data-driven approach ensures that high-risk individuals are prioritized for referral to Janakeeya Arogya Kendrams (JAKs). Here, Junior Health Inspectors, Junior Public Health Nurses, and Mid-Level Service Providers utilise the JAK App and Public Health Portal to validate screenings and manage referrals efficiently.
The ecosystem is designed to ensure a seamless continuum of care. Patients identified as screen-positive are directed to designated Primary or Family Health Centres for confirmatory diagnosis and treatment initiation via the eHealth Hospital Management System. By leveraging the eHealth UHID, the platform ensures systematic longitudinal follow-up ensuring Continuum of Care at both the sub-centre and primary care levels. This end-to-end digitisation not only optimizes patient management but also facilitates the accurate mapping of disease prevalence, enabling timely and targeted public health interventions.

The initiative expanded under SHAILI2.0 to include screening for other diseases like such as tuberculosis and leprosy, as well as mental health challenges, visual and hearing impairments, and geriatric health issues.
Moving beyond screening, Phase 3 establishes a robust continuum of care by integrating data with the eHealth Hospital Management System (HMS) and JAK app to facilitate Screenig, systematic treatment, patient follow-up, and comprehensive family health mapping.SHAILI 3.0 was inagurated by Hon'ble Cheif Minister of kerala, on January 1st 2026
